So, where is this damage occuring?

I recommend you to test at least 2 other vendors for the diode; you can find that the problem is diode manufacturing, or else, you will find out that you have a proccess problem for sure.

There is no use to start investigating your proccess before you are sure the problem is there !

The diode doesn�t appear completely broken. It takes a bit white color (cristal break, I think), instead of the normal red/orange. It works like a 20K-200K ohm resistor. !The diode can be working properly and sometimes change to bad!

I think it may be a problem of board flex. Is it normal? I have seen it in SMD capacitors (KEMET flex crack files) in our products.
